BRADFORD D. CROFT,
D.O., M.B.A., F.A.C.O.F.P.
Supervising Provider
Dr. Bradford Croft is retired from patient care, but currently serves as our supervising provider. He has been a trusted physician in Northern Arizona since 1981, bringing over four decades of experience in family medicine and osteopathic care.
Dr. Croft earned his B.S. cum laude from Dickinson College in 1975, followed by his D.O. from the Philadelphia College of Osteopathic Medicine in 1980. He completed his post-doctoral internship at Phoenix General Hospital in 1981 and later earned an M.B.A. from the University of Phoenix in 2004.
He opened his Flagstaff-based family practice on July 1, 1981, and became board-certified in Family Practice and Osteopathic Manipulative Treatment in 1990. In recognition of his dedication, he was named a Fellow of the American College of Osteopathic Family Physicians (ACOFP) in 1996.
Dr. Croft is an active member of the American Osteopathic Association, ACOFP, and the Arizona Osteopathic Medical Association, where he has served as a delegate since 1986.
SARA REEVES, FNP
Sara Reeves is a Certified Family Nurse Practitioner with 17 years of diverse nursing experience, specializing in children’s health, family care, and public health. She is dedicated to providing compassionate and personalized care to her patients.

Sara combines her extensive expertise with a warm, patient-centered approach. Her broad skill set allows her to effectively address a wide range of health issues, ensuring comprehensive care for her patients.
In addition to her primary care services, Sara offers Botox treatments, achieving natural-looking cosmetic results and providing relief for medical conditions such as migraines and excessive sweating.
In her free time, Sara enjoys fishing, playing pool, swimming in the ocean, and gardening. These hobbies reflect her love for the outdoors and her zest for life, qualities that she brings into her daily practice.
MEREDITH SMITH
FNP-C, ACNPC-AG
Meredith Smith is a Certified Nurse Practitioner with 18 years of nursing experience. Throughout her career, she has excelled in various roles, including ICU and Surgical Services, demonstrating her versatility and dedication to patient care.

Her professional journey has been marked by significant contributions to disaster relief efforts and surgical campaigns in Haiti and Bolivia with NAVMC, showcasing her dedication to global health and humanitarian work.
Passionate about preventative medicine, Meredith is eager to work with patients to promote healthy living and prevent disease. Outside of her professional life, she enjoys hiking and running with her dogs, cross-country skiing, traveling, and gardening.
Meredith looks forward to bringing her wealth of experience and compassionate care to the community.
BROCK VENMAN PA-C
Brock Venman is a dedicated Physician Assistant with a strong background in exercise science and healthcare. He is enthusiastic about the opportunity to partner with patients to promote healthy lifestyles and help treat and prevent disease.

After growing up in Ohio, Brock is excited to start a new chapter in Flagstaff, Arizona, where he looks forward to exploring the beautiful outdoors. Outside of work, he enjoys playing soccer, running, skiing, and spending time with his puppy.
NATALIE MARCIALES PA-C
Natalie Marciales is a dedicated and certified Physician Assistant with a strong foundation in public health and a passion for patient care. She has developed a deep understanding of community health and preventive medicine through her education and professional experience.

Natalie has worked in rheumatology, where she excelled at diagnosing and treating autoimmune arthritis. Her expertise in this specialty equips her with valuable skills for managing chronic conditions and improving patients' quality of life.
Natalie is known for making patients feel comfortable and her dedication to providing thorough patient education. She believes informed patients are empowered to make better health decisions and takes pride in ensuring her patients understand their conditions and treatment options.
Outside of work, Natalie enjoys reading, birding, and traveling.
Natalie is excited to bring her expertise and compassionate care to East Flagstaff Family Medicine, where she looks forward to building strong relationships with her patients and contributing to their health and well-being.
Dwight Jones FNP-C
Dwight Jones brings over 15 years of diverse healthcare experience to East Flagstaff Family Medicine, with a background that spans critical care, emergency medicine, education, and military leadership. Dwight’s clinical training includes family medicine, urgent care, pediatrics, OB/GYN, orthopedics, and pediatric urology—giving him a well-rounded foundation to care for patients of all ages.
Education:
He completed his Master of Science in Nursing as a Family Nurse Practitioner at the University of Missouri-Columbia in December 2024, following a distinguished career as an ICU nurse.
A U.S. Army National Guard veteran, Dwight’s leadership skills were honed through six years of military service. His advanced assessment and triage capabilities, along with his calm demeanor and strong critical thinking, make him a trusted provider for both routine care and complex cases. Outside of medicine, Dwight enjoys backpacking and holds a bachelor’s degree in Fisheries and Wildlife. He is excited to serve the Flagstaff community with compassionate, evidence-based care.
Emily Gill
Practice Manager
Emily serves as the dedicated Practice Manager for East Flagstaff Family Medicine, where she combines her passion for healthcare with her commitment to the local community. Her love for managing this locally owned clinic is evident in the personalized care and attention she ensures every patient receives.
She graduated from Northern Arizona University with a bachelor's degree in Public Health in 2020. Beyond her commitment to healthcare management, Emily enjoys fitness, hunting, fishing, reading, and gardening, balancing her professional responsibilities with a passion for the outdoors and personal enrichment.
Elise Guzman
Assistant Office Manager
Elise is an Arizona native with a deep-rooted love for the outdoors and people. Growing up in Prescott, she developed a strong connection to nature. Her passion for the environment led her to pursue a Bachelor of Science in Forestry at Northern Arizona University, which brought her to Flagstaff.
Before moving to administrative patient support and industrial medicine, Elise was involved in non-profit work involving public outreach and education, watershed health, ecology, and recreational impacts in Northern Arizona.
Elise finds solace in the great outdoors. Hiking, camping, and exploring off-road trails with her canine companions are her favorite pastimes. Elise's unique blend of education, experience, and enthusiasm makes her a great fit for our practice!
Jered Richards
Lead Medical Assistant
A Flagstaff native, he brings a wealth of diverse medical experience to his job. His foundation in healthcare was built during eight years of dedicated service in the United States Navy Reserves as a Hospital Corpsman. While in the service, he honed his skills as a surgical cleaning technician, a dental assistant, and a field medical technician.
He further developed his patient care abilities as a Tier 1 phlebotomist at Sonora Quest, becoming proficient in laboratory procedures. For the past five years, he has been a committed member of the team at East Flagstaff Family Medicine, serving their local community.
Mike Cintron
Lead Front Office
Mike is the lead front desk coordinator with a passion for people and positivity. A proud triple-degree graduate from UCLA, he brings both intelligence and heart to everything he does—from serving as a youth pastor to brightening days with his signature fun shirts (and a splash of pink on Wednesdays).
When he's not assisting our patients, Mike enjoys reading, drawing, and spending quality time with his beloved dog and family
